Fill out the form below to access your care team, schedule an appointment, and manage your primary care account through the Hello Health platform.
If you have been provided with an Activation Key:
- Fill in the form below, agree to the “Terms and Conditions,” and click Submit.
- You will then be redirected to the login page. Enter your email as your Username, and the Password you just created to access your account.
- In the pop-up window, select your primary care provider.
- Select the Essential Health Connection Plan you would like to subscribe to, review and agree to the Hello Health Terms and Conditions.
- Review the description of the fee that will be charged to your account. Select “Subscribe.”
- Enter your payment information. Select “Confirm.”
- VERY IMPORTANT FINAL STEP: Access “My Profile” to update your Contact Information. Enter your email address, and change your password to something unique and memorable for you. This email address will be used to notify you of activity on your secure Hello Health account.
